MRI and firearm discharge cites sought

A while back, there was a report about an accidental discharge (AD) of
a law enforcement officer's duty weapon when it was accidentally
brought into the magent room. I know I saw a report on it, but can't
remember where it came from. Does anyone have some cites on this
incident? Or at least enough information to whittle down the results
from my Google search?

We are attempting to determine what might actually have happened,
since the firearm in question is almost completely incapable of firing
in the manner described, according to my memory. So I'd appreciate it
if anyone can jog my memory a little.

I used to think the horse's name was Bob.
And the angel's name was Hark.
