What is the advantages of the 3/4D Ultrasound in detection of conjoined twins, and what can be done

3D static or real time (4D) has simplified the detection of this form
of twinning which is monoamniotic or monochorionic monozygotic.
The 3D acquisition can pick information in different planes that
cannot be achieved by the simple 2D exhaustive search for the
deformities present in this form of conjoined twins.
This patient was referred to us from her obstetrician as a case of
polyhydramnios with a mass connected to the anterior abdominal wall of
the fetus and his provisional diagnosis was probable exomphalos
major.
With the help of 3D the exam can provide the following information:
1. The junction between the two fetuses.
2. The extension between the two fetuses.
3. The regional distribution of extension.
4. The continuity of the organs involved in the process of sharing
(thoraco-abdominal wall sharing, liver sharing, great vessels
sharing).
5. Other defects (exomphalos in one of the two fetuses and liver
sharing and great vessels sharing).
6. The spinal deformity of severe lordosis in both fetuses as a
compensatory mechanism of weight bearing (every fetus is trying to
uplift or carry his co-twin).
The information will help the team of management of choosing the
method of delivery and possibility of surgical correction.
