Scar Tissue around patella

I had a total knee replacement in May 2007. I have swelling and don't
have full extension of the knee. My surgeon tells me that I have
scar tissue wrapped around the patella. He is very concerned about this
and doesn't understand how it formed as I was totally
committed to my recovery. For the first two months, I had a physical
therapist that caused me so much pain during my sessions
that I would cry after every appointment. He went on vacation and I
then had an excellent PT who worked me but not to the extreme.

I've done acupuncture, swimming, elliptical, massages and just can't get
better. I have lots of swelling after exercise.

Two questions:

Could this first therapist have caused the scar tissue by working my
knee too hard
What can be done to break up the scar tissue
